The Report Center may be defaulting to the first Record Form entered. Make sure the correct Record Form is being selected when setting your report criteria.
OR
The changes to the record form were not committed. Changes within the 90 days must be committed for them to take effect for reporting purposes.
Pro Tip: You can see how many days you have left to edit a record form under the Days to Edit column when viewing child information.
